After scheduling your appointment, please complete the Health Assessment and Bach Flower Essence Questionnaire forms and return them to Shannon along with your DNA samples.

DNA Samples for Testing:

Please send a Q-tip lightly dampened with saliva, a few fingernail clippings, and a small snip of hair. Place each sample in individual baggies, labeling them with your name and the date using a Sharpie. Send the samples to:
